Question 1: Why did I receive a Notice of Violation?
You, as the registered owner of the vehicle described on your Notice of Violation, are alleged to have violated the state and/or local law(s) applicable at the location of the violation. For more information, please see the top portion on the back page of your Notice of Violation.
Question 2: Will this be reported to my insurance or go against my driving record?
No points will be assessed on your driver’s license for this Notice of Violation.
Question 3: How much is this fine?
Violation fines differ by state and/or local law(s) applicable at the location of the violation. For fine amount, please see the top portion on the front page of your Notice of Violation. Failure to pay may result in additional fines, penalties and costs.
Question 4: How do I pay my Notice of Violation?

Online Payment: Pay online at the Child Safety Program Website: https://www.childsafetyprogram.us

Phone: Call toll-free 1.800.457.1788 between 8:00 AM and 5:00 PM CST, Monday through Friday.

Mail: Mail your cashier’s check, personal check or money order (payable to Child Safety Program) in the envelope enclosed with your Notice of Violation, along with the cut-off portion printed at the bottom of the reverse side of the notice.

Payment options the day of court may vary as allowed by the particular court.
NOTE: The time period to pay is shown on the reverse side in the box on the top right corner. There is a 3% convenience fee charged by the banking institution for credit card payments (by phone or online).
NO CASH
Please refer to the bottom portion of your Notice of Violation for instructions on mailing your cashier’s check, personal check or money order. Child Safety Program will not accept cash. If you need further assistance, please call 1.800.457.1788 between 8:00 AM and 5:00 PM CST, Monday through Friday.
Question 5: How do I contest a violation?
If you would like to contest or appeal your Notice of Violation, you must do so by the due date located on the top right of your Notice. If you have not received email notification of the status of your case within 48 hours, please call 1.800.457.1788 between 8:00 AM and 5:00 PM CST, Monday through Friday. Be sure to have your Access Code and PIN ready for your convenience.
Conditions for Possible Waiver or Refund of Fine
Your fine may be waived if one or more of the following exceptions applies to you and you have submitted an Affidavit.

Name the Driver at the Time of the Alleged Violation
Submit a sworn, notarized statement that you were not the operator of the vehicle at the time of the violation, providing the name and address of the driver, by completing the Affidavit and mail to the address below.

Vehicle or Plate Reported Stolen Prior to Alleged Violation
Please complete the Affidavit and mail to the address below. Upon review, you will be notified of the decision.

Dispute/Appeal/Rebuttal of the Violation by Appearing in Court
To request a hearing, sign the appeal request and mail it to the name and address on the reverse side of your Notice. You will receive a hearing date in the mail with the date, time and location of your court date.
